The Wellness in Wallaceburg Educational Campaign spotlights services provided by local organizations in your community that are working to improve your health and wellbeing.

This week’s spotlight is on the Chatham-Kent Community Health Centres.

What services do you provide?

At the Chatham-Kent Community Health Centres (CKCHC), our interdisciplinary team delivers primary health care in combination with health promotion and community development services all under one roof.

At our community health centre we promote the health & well-being of individuals & families, but also mount initiatives that address social, economic & environmental problems that are negatively impacting people’s health.

Who can access your services and is there a cost?

Registered clients can come to the CKCHC to have their regular health care needs looked after by a team of doctors, nurse practitioners, nurses, dieticians and other health providers.

The community programs offered at CKCHC are free and open to all community members. These programs include diabetes prevention, fitness, youth programs, chronic disease management, nutrition, addictions and aging-at-home services.

How do people access your services?

To access CKCHC’s interdisciplinary team one-on-one, potential clients need to fill in an application.

This application is then reviewed, and once accepted, the client will have access to a physician or nurse practitioner as well as our allied health team. To be accepted as clients, individuals need to belong to one of our priority populations.

Priority is given to people who do not have a primary care provider and who may have difficulty obtaining health services.

For the aging-at-home programs and services, such as COPD support and falls prevention, clients must fill in a referral or have their primary care providers fill in a referral.

What is the future direction of services in the Wallaceburg area?

In the past year the CKCHC provided access to over 21,000 primary care visits.

For Wallaceburg the goal is that services will continue to expand with a focus on increased access to primary care, disease prevention, health promotion and wellbeing.

What’s the best advice you can give to people to help them stay well?

“Good health is much more than something you get in a medical clinic. Better health begins in the community where we live” (AOHC, 2014).

On October 21st in Wallaceburg, there will be a Diabetes Prevention Info Session that includes light refreshments from 12-1pm.
