The Wellness in Wallaceburg Educational Campaign spotlights services provided by local organizations in your community that are working to improve your health and wellbeing. This week’s spotlight is on the Erie St. Clair Community Care Access Centre (CCAC).
What services do you provide?
The Erie St. Clair CCAC helps patients access the care they need to reach their health goals.
In addition to connecting patients to valuable supports and resources in the communities of Chatham-Kent, Sarnia-Lambton and Windsor-Essex, we also provide a wide range of health care services to eligible patients.
These services include nursing, personal support, physiotherapy, occupational therapy, speech therapy, social work and nutritional counselling. When living at home is no longer possible, we also assist people in the transition to long-term care. Who can access your services and is there a cost?
Anyone can call the Erie St. Clair CCAC for information on health and social services in the community, as well as to learn more about our services and self-refer.
Our services are for people of any age who are assessed as eligible.
The services we provide to eligible patients are free of charge to those with a valid Ontario Health Card.
The Ministry of Health and Long-Term Care provides the funding for these services.
However, there may be other costs associated with a patient’s care such as medical equipment and community program fees.
How do people access your services?
Anyone can access our services by calling the Erie St. Clair CCAC at 310-2222 (no area code).
A comprehensive health assessment is performed to determine a patient’s needs. Based on this assessment, a care plan is developed to determine which services you require to meet your health goals.
What is the future direction of services in the Wallaceburg area?
The Erie St. Clair CCAC is committed to providing increasingly patient-centred care across Chatham-Kent, Sarnia-Lambton and Windsor-Essex.
We will continue to help people navigate the health care system in their community and manage living with chronic disease.
What’s the best advice you can give to people to help them stay well?
The Erie St. Clair CCAC encourages everyone to take an active role in their health care. This includes being aware of the supports and services available in your community.
