Chatham-Kent Ontario Health Team launches new five-year strategic plan

The Chatham-Kent Ontario Health Team has unveiled its 2025–2030 strategic plan, a roadmap aimed at improving health outcomes and building a more connected, inclusive health system in the region.

The plan, shaped by more than 200 partners and 600 community members through surveys, workshops and facilitated sessions, focuses on strengthening primary care, fostering accountability within the health team, and embedding equity and inclusion into all aspects of care.

“Our strategic plan is a testament to the power of partnership,” said Melissa Sharpe-Harrigan, executive transformation lead with the health team. “It represents the voices of our community, our providers, and our partners — all working together to build a healthier, more connected Chatham-Kent.”

The health team said the process was built on broad collaboration to ensure a wide range of perspectives were included, from clinicians and care providers to local residents.
