Chatham-Kent Health Alliance’s (CKHA) Diabetes Education Centre has recently launched new services, aiming to enhance the standard of care for individuals living with diabetes.
“These new physician-led services will be an invaluable asset to CKHA’s Diabetes Education Centre, contributing to our multidisciplinary approach to patient care and collaborating closely with our existing team of healthcare professionals,” said Dr. Zeke Milkovic, Acting Chief of Staff, CKHA, in a media release.
“CKHA is committed to providing comprehensive care to individuals living with diabetes,” added Caen Suni, Vice President, Clinical Programs and Operations, CKHA, in a press release.
“These services will address various aspects of health for this chronic condition, ensuring that patients receive the necessary resources and support to handle and avoid complications associated with this disease.”
New services provided through CKHA’s Diabetes Education Centre include:
Monthly Clinic for Patients without a Primary Care Provider
A monthly clinic will be provided by Dr. Kamran Karatela for patients with Type 2 Diabetes (with complications) who don’t currently have a Primary Care Provider. The patient will be seen by a Certified Diabetes Education (CDE) Certified Registered Nurse and/or a Dietician and Dr. Karatela to help address their diabetes needs and to provide patients with essential knowledge and skills for managing their condition effectively.
Gestational Diabetes Clinic
Dr. Simone Mendel will be available for consult for patients with Gestational Diabetes. CDE Certified Registered Dietitians will work collaboratively with Dr. Mendel and offer personalized nutritional counseling sessions to help patients develop healthy eating habits and assist with the initiation of insulin when needed.
Dr. Ibrahim Treki, Endocrinologist
Dr. Treki is accepting new patients with the following conditions:
– Type 1 Diabetes Adult and Transitioning Paediatric to Adult
– Insulin Pump Therapy
– Diagnosis Clarification
– Discharged patients from hospital who are experiencing unstable blood glucose patterns, diabetic ketoacidosis (DKA) and/or hyperglycemic hyperosmolar state (HHS)
– Glucocorticoid induced hyperglycemia
– Uncontrolled Type 2 Diabetes despite treatment
– Diabetes in pregnancy and pre-conception counseling
Patients who qualify for these services will be referred via their family physician, local walk-in clinics and through CKHA’s Emergency Department and Ambulatory Care Department.
