What services do you provide?

The Chatham-Kent Family Health Team provides primary care to over 35 000 patients through our Physicians and Integrated Health Care Providers. Urgent Care Services are also available after hours several nights a week. The Family Health Team has over sixteen wonderful Programs/Services to offer the patients of the FHT including Balance Assessment, Counselling, Cardiac Education, Chiropody and Footcare, Cognitive Screening, Chronic Disease Management, Diabetes Wellness Program, Dietary Counselling, Ergonomic Assessment Services, Falls Prevention, Healthy You, Medication Therapy Management, Respiratory Program, Return to Work, Smoking Cessation and Women’s and Men’s Health Services. It is through these many Programs and Services that we are “Building Healthier Families Together”.

The patients of the Chatham-Kent Family Health Team.

How do people access your services?

People can talk to their Primary Care Provider, or self-refer to the CKFHT Programs.

Is there a cost?

There is no cost to the patients for these Programs and Services with the exception of a small fee for Footcare Services.

What is the future direction of services in the Wallaceburg area?

The Chatham-Kent Family Health Team is very excited to have recently added three family physicians to the Wallaceburg site.

Also, renovations to the clinic means that these Programs/Services are offered in Wallaceburg.

What is the best advice you can give to people to help them stay well?

Much of the research around staying healthy stresses that people focus on staying active, eating well, quit smoking, maintaining a healthy weight and connecting with their primary care provider to take part in preventative testing.
