Community Health Centres purchase new accessible van

The Chatham-Kent Community Health Centres (CKCHC) has recently purchased a new, accessible van for the provision of home visits and outreach services in the community.

The purchase of the new van would not have been possible without the generous support from Farm Credit Canada and a generous donor, CKCHC officials say.

The CKCHC was awarded funding through the AgriSpirit Fund in September 2020.

“We are grateful for the support from Farm Credit Canada coupled with a previous donation towards a mobile unit from a generous donor which has allowed us to replace the aging van and continue to provide many important services throughout our community,” stated Sherri Saunders, executive director for the CKCHC, in a media release.

The CKCHC offers a number of programs and services throughout Chatham-Kent.

The Age Well program aims to keep individuals safe and thriving in their home environment by offering home safety assessments, individualized home exercise programs, and health education for chronic disease management. The van allows the program staff to bring these services directly to the client’s home.

The van is also used to support CKCHC team members and clinicians to provide home visits and a wide variety of outreach and other health promotion programming, CKCHC officials say.

When transportation is a barrier to receiving care, the van can also be used for clients who lack access to other transportation options, CKCHC officials added.
