ConnectMyHealth online portal now available

A new online patient portal called ConnectMyHealth is now available for the public to access their personal health records across multiple participating hospitals in southwestern Ontario, including Chatham-Kent Health Alliance.

It is free, open to anyone aged sixteen and older, and can be accessed from a desktop computer, laptop or mobile device, officials with the Chatham-Kent Ontario Health Team say.

There is no app to download in order to use it.

Once registered, individuals will be able to view various types of health records, including lab results, diagnostic tests such as xrays, MRIs and ultrasounds, and more.

Individuals are able to receive email notifications when new results are ready to be viewed in their health record, and are also able to print their health records to easily bring to appointments and for personal record keeping.

ConnectMyHealth is meant to act as a tool that patients can leverage to access their health records, understand their health care, and improve their experience, Chatham-Kent Ontario Health Team officials added.

It can be a beneficial tool for anyone, however it can be especially empowering for individuals who have been or are currently receiving care at a hospital, such as those undergoing cancer treatments, and those with chronic health conditions needing to access and share past records of care, or plan for follow-up with their family doctor.

“I recently had to have some follow-up testing done at a hospital out of town,” said Kathy Borthwick, Chair of CKOHT’s Patient and Family Advisory Council, in a press release.

“I had the tests done, and then received an email the next day that the results were ready to view in ConnectMyHealth. Early access to the reports helped me prepare for the follow-up appointment so that I could better participate in the discussion with the doctor.”
