Chatham-Kent Ontario Health Team launches strategic plan (Video)

The Chatham-Kent Ontario Health Team (CKOHT) has officially launched its inaugural strategic plan 2021-2024.

“We are entering an era of opportunity, to come together to make positive change for our residents, patients, caregivers and their families,” stated Melissa Sharpe-Harrigan, executive transformation lead for CKOHT, in a media release.

“We have set five bold goals to help us improve health care in many innovative and collaborative ways in Chatham-Kent by 2024.”

Chatham-Kent Ontario Health Team is a partnership committed to co-designing a local health care system with improved access to care that is easier to navigate for everyone.

Their vision is achieving the best health and wellbeing together.

CKOHT officials say this strategic plan was built from input from over 1,700 residents in Chatham-Kent, and 100s of health, social and community support services leaders.

Their current priority population is focused on those aged 55+ with chronic and complex health conditions.

They are also expanding the focus to include mental health and addictions.

“We have worked very hard to create a strategic plan that has been guided by patients and healthcare providers and look forward to leveraging our strong relationships within our Chatham-Kent Ontario Health Team to achieve our goals over the next three years,” stated Dr. Briana Yee-Providence, physician co-chair for CKOHT, stated in a media release.

“Transforming care at the local level we will focus on navigation of the healthcare system with improved access to care that is patient focused and strives for equality.”
